1. New Patient Registration Form
2. Notice of Privacy Practice and Acknowledgement
3. General Office and Financial Policies
4. Your Child’s Age and their Privacy

Plus, the appropriate HIPPA Form below:

• HIPPA Authorization Form for Parents
• HIPPA Formulario de Autorización para Padres
• HIPPA Authorization Form for 18 and Over
